Family medicine clinics provide 'medical home' for patients | The ...

As a patient with multiple sclerosis who sees a number of doctors, Christine Abbott used to feel a disconnect among them about her health care. About four years ago, though, she started going to the Family Medicine Clinic at Medical College of Georgia Hospital and Clinics, where her providers began talking to one another and sharing information.

?When I go into Family Medi?cine, they have the whole picture of me, which my community physician did not have,? Abbott said. ?I?m more confident that everyone is on the same page.?

Abbott and others benefit from the clinic?s embrace of patient- and family-centered care and from a concept called ?patient-centered medical home,? a distinction a clinic can earn from the Na?tional Committee for Qual?ity Assurance for meeting standards for access, care management, outcome monitoring and quality of care.

There are 4,220 practices in the U.S. that have earned the distinction, with 26 in Georgia and three in Richmond County: Family Medicine at GHSU, Family Medicine at Fort Gordon and the Center for Primary Care.

Practices have to meet those standards to be called a patient-centered medical home, but it is not an end in itself, said Dr. Joseph Hobbs, the chairman of Family Medi?cine at GHSU.

?To me, patient-centered medical home is a tool that was used for us to be able to deploy effectively patient- and family-centered care and to deploy effectively population-based care and management and be responsible for all of those things,? he said. ?The PCMH is a tool that permits us to apply those concepts to our practices because we believe those concepts can improve not only patient satisfaction but the quality of services we deliver, and health outcomes in general.?

Complete care

The concept of patient-centered medical homes first surfaced in pediatrics in the 1960s and was more recently embraced by primary-care associations as the need for continuity of care became more apparent, said Dr. Frank Don Diego, the academic chairman and program director of the Floyd Family Medicine Residency Program in Rome, Ga. It is a comprehensive, team-based approach in which the practice takes on the responsibility for coordinating and improving care, he said. That means thinking about the patient outside of the exam room and beyond the last appointment, Hobbs said.

Physicians, nurses, front-office workers and everyone else in the office must be retrained to take on different roles in providing and coordinating that care, Don Diego said.

?It does take a lot of training,? he said at a recent meeting of the Association of Health Care Journalists. ?It does take a lot of teamwork.?

For instance, practices usually have to hire a care coordinator who tracks the records and keeps an eye on the bigger picture, even if it is not reimbursed care, Don Diego said.

?We?re doing a lot of work without? reimbursement, he said.

That is part of a larger problem with primary care in general, said Dr. David Satcher, of the Satcher Health Leadership Institute at the More?house School of Medi?cine.

?Primary care has been, in our system, devalued,? he said. ?And now we?re paying a price for that.?

There had been plans for enhanced fees for patient-centered medical homes but that has yet to happen and should not be the motivation for pursuing the designation, Hobbs said.

?My motivation was the realization that in order to appropriately accept the responsibility of the population of patients that is assigned to us, that we needed to put together a different infrastructure that would use new tools and maximize the functionality of old tools to provide us data about how well we did,? he said.

Cutting costs

The clinic at GHSU has had an electronic health record for years but wasn?t fully taking advantage of all its functions, Hobbs said. The clinic is working on getting patients greater access to their health data through an online portal, but for some that won?t be appropriate and they will continue to get mailed information to help them manage their conditions, he said.

Studies show that people with chronic conditions usually receive about two-thirds of the care they need, in part because of disparate care, Don Diego said.

Providing better care through a patient-centered medical home has been shown to reduce hospitalizations and visits to the emergency room, which reduces cost, Don Diego said.

?This is dollars,? he said. ?This is why people want this.?

That drives clinics to make the difficult transition from focusing on the volume of patients seen to focusing on patient outcomes, Hobbs said.

?You?re being evaluated by the quality of services you deliver,? which is what patients expect, he said.

For Abbott, it is as simple as her primary care doctor knowing that one of the neurology drugs she takes has a side effect of causing diabetes in some patients.

?(The doctor) can start keeping an eye on the diabetes aspect and help me with counseling and the things I need to do,? she said. ?Everything I take has a side effect. Most of it, I can manage with diet and exercise. They are much more in tune to what I need personally.?

Johan Santana No-Hitter: Mets Pitcher Throws First No-Hit Game In Franchise History (VIDEO)

Johan Santana has thrown the first no-hitter in the history of the New York Mets. The 33-year-old ace of the Mets' staff went the distance against the visiting St. Louis Cardinals at Citi Field without surrendering a hit in an 8-0 win.

Through eight innings, Santana had thrown 122 pitches while striking out seven and walking five. He faced the heart of the Cardinals' order in the top of the ninth, retiring Matt Holiday, Allen Craig and David Freese to make history in Queens. All told, he threw 134 pitches. The final pitch of the game was yet another signature change-up from the two-time Cy Young winner that Freese swung over.

Speaking with Kevin Burkhardt of SNY on the field after the final out, the first word uttered by Santana was "amazing." For a franchise described lovingly as "Amazin" even if the results were intermittently anything but, it was a choice of words as spot on as his final pitch.

"Amazing. I mean, coming in to this season, I was just hoping to come back. You know? And [to] stay healthy and help this team. And now, I'm in this situation in the greatest city for baseball," said Santana before addressing the fans at Citi Field. "I'm very happy and happy for you guys. Finally! The first one!."

Asked about the final strikeout, Santana said it was "the greatest feeling ever."

There were tens of thousands of fans in Queens and potentially millions more at home (and on Twitter) who felt the same way. The franchise had previously played 8,019 games -- stretching back to April 11, 1962 when Casey Stengel gave the ball to Roger Craig for the first game in club history -- without adding a no-no to its resume.

Entering the season, Santana's future was uncertain. Before the team's season opener, Santana had not appeared since Sept. 2010 and had undergone serious shoulder surgery.

The visiting Cardinals, on the other hand, entered the season as the defending World Series champions. Despite losing cornerstone hitter Albert Pujols via free agency, the Cardinals arrived in New York with the highest team batting average and home run total in the National League.

The historic result depended on a few critical plays in the field as well as one potential umpiring blunder. In the seventh inning, outfielder Mike Baxter made a spectacular catch as he ran into the wall. More controversially, Santana appeared to get the benefit of an incorrect ruling a fair ball to be foul during the sixth inning when former Mets star Carlos Beltran appeared to rifle a ball fair up the third-base line.

"It was tough because it happened so quick," Santana told reporters after the game when asked about the play. "I wasn't able to see anything. They just called it "foul" and I just stayed with it. There are times when one play makes the whole difference, when one call makes the whole difference. And if that was the case tonight then that was the call."

That the key play that went the Mets way involved Beltran added another layer of intrigue to the night as his relationship with the fans was never as steady as his performance while with the team. The low point of Beltran's six plus seasons came against the Cardinals in Game 7 of the 2006 NLCS in Queens when St. Louis pitcher Adam Wainwright froze him on a nasty curve ball to end the game. Coincidentally, Wainwright was opposing Santana on Friday. Thanks to the call by third base umpire Adrian Johnson those fans won't #BlameBeltran after this historic night.

The 8-0 win lifted the upstart 2012 Mets to 29-23 on the season under manager Terry Collins, who will be nervously watching Santana in the coming days to see if the heavy workload takes a toll. Lucas Duda and Daniel Murphy did the heavy lifting for the Mets' offense, with four and three RBI respectively.

From Tom Seaver and Nolan Ryan to Doc Gooden and David Cone, the Mets have sent some of the most dominant pitchers in MLB history to the mound during more than fifty years in New York but until Friday night the club never had a pitcher toss a no-no.

Sean Dougherty Returns to Brand New School as CD and Director ...

Twelve years ago when Brand New School opened its doors in Venice, California, Sean Dougherty was one of its first employees. He became the company?s senior art director through his leadership and his vital contributions to projects for MTV, FUEL TV and CMT, to name but a few, and when BNS expanded to New York City, Sean was there, too? all the way through to 2007, when he went freelance. BNS has recently added Sean back as a creative director and director.

Since 2010, Sean has served as associate creative director and director for Superfad in New York. There, his collaborations with executives from leading advertising agencies including Euro RSCG Worldwide, Wieden+Kennedy, Young & Rubicam and many others have produced groundbreaking campaigns for Target, Starwood Hotels, Ritz Crackers, Macy?s and Bloomberg, among many others. He also directed a seven-minute segment of ?A Liar?s Autobiography 3D,? the stereoscopic 3D feature film on the life of Graham Chapman from Monty Python.

In between his time at Superfad and BNS, Sean continued his art direction and commercial director endeavors. As a founding partner of design house Golden Lucky (representation through London-based Rokkit), Sean directed US and European commercials for FUEL TV, McDonald?s and Virgin Media. While working as a freelance art director for Imaginary Forces, National Television and eventually Superfad, Sean guided visual strategies for countless entertainment networks, electronics and automotive brands, and many other major accounts.

Born in Orange, California, Sean began his studies at the University of California, Irvine, before earning his BFA in Advertising/Graphic Design from Pasadena?s Art Center College of Design.

?Brand New School is both familiar and new to me,? Sean explained. ?When Jonathan showed me around the current studio, I got excited to see the variety of projects being produced and the resources applied to each one. I also know that Jonathan and I have a good working history together that can only be enhanced by the fact that we?ve had other separate experiences for the last few years. In the days ahead, I?ll be focusing on being a nurturing and supportive leader as a creative director, and I?ll also be continuing to push forward with directing spots and projects.?

Life, Interrupted: Keeping Cancer at Bay - NYTimes.com

?How are you doing?? is a complicated question for a cancer patient.

Watching for the return of cancer after a bone marrow transplant is a full-time job, even when results from the first biopsy seem hopeful. Last weekend, my doctors gave me a ?pass? to leave the Hope Lodge, my temporary home in Manhattan, for a few days. It was good to have a break to travel upstate to my parents? house. Unfortunately, I got sick to my stomach during the four-hour trip and spent most of Friday and Saturday sleeping in my childhood bedroom.

But I couldn?t help thinking of Memorial Sloan-Kettering Cancer Center, the New York City hospital where days earlier my doctor had delivered the results of my first bone marrow biopsy since my transplant. If you had asked me how I was doing last Thursday morning ? crossing Manhattan in a cab on a muggy, drizzly morning on my way to receive the test results ? I would have described the sense of dread welling up inside of me. The biopsy would be the first glimpse at my new bone marrow, the foundation of my new immune system, which had been ?rebuilt? with my brother Adam?s healthy stem cells during my bone marrow transplant last month. Would the cancer still be there?

At my appointment, my doctor announced he had two pieces of news. The good news was that my biopsy showed no cancerous cells in my bone marrow; the second part of the news was that I had to begin chemotherapy again immediately to keep my cancer from returning.

The CT scan. The biopsy. The X-ray. The waiting. These tests, at once commonplace and unnerving, are the trusty siblings of cancer treatment. For every round of chemotherapy, for every procedure, there?s a test that determines how well it worked. In between, there is time to wait and not much else to do. Bad news can always be just over the horizon. And good news rarely comes without qualifications.

On Sunday, up at my parents? house, I mustered the energy to walk to the next-door neighbors? Memorial Day barbecue. Wearing the face mask I need in public, I sat on a lawn chair in the corner of the backyard, next to my brother Adam, my bone marrow donor. Adam reminded me that it is his job to ?police? my face mask use, as even the most common germs can be deadly to my fragile immune system. I slipped bites of cantaloupe, which my mother had prepared specially for me to ensure it was sanitary, under my mask as Adam looked on, smiling. ?Maybe you should have a pouch for food on the inside of your mask,? he joked.

Neighborhood children ran through a sprinkler at the center of the backyard, dripping wet and shrieking happily. Their parents gathered around the food tables, some drinking the mint juleps that my neighbor Shane had prepared. My parents stayed close by, chatting with friends but fretting about whether I was getting dehydrated from the heat. My mouth watered for the six different salads at the nearby table ? I counted them ? couscous with raisins, quinoa salad, fresh arugula and the rest of them, but I knew I wouldn?t be able to have any for fear I could get sick from any bacteria that might be in them.

I was enjoying the breeze. A neighborhood girl named Calla, the 9-year-old daughter of one of my father?s university colleagues, came up to me to say hi. The last time she had seen me I had long hair.

?Why are you wearing a mask?? she asked.

?Because I?m sick,? I said.

?Why are you sick??

When I didn?t answer, she paused for a while, then suggested I paint on the outside of the mask, or draw a mouth on it, ?like Halloween.?

I?ve begun to think of cancer as a junkyard dog. It may be fenced in, but it?s snarling, always trying to dig under the fence. So the tentative news was good. My bone marrow was clear, but I would have to work like hell to keep it at bay, behind the fence. One test can determine an initial diagnosis, but it?s many tests over months or years that track the progress to a cure. That means there?s always another scan down the road. The biopsy next time.

Smoke rose from the grill, where my neighbor Jamie was cooking the pork belly that everyone?s been raving about, past the hedge between the houses and off toward my bedroom window of my parents? Victorian-style house, where my family moved when I was 12.

Cancer is always lurking in my ZIP code, whether I?m in Manhattan or in upstate New York. Today is Day Plus 50 ? 50 days since my transplant ? which means I?m halfway to Day Plus 100, the critical period for bone marrow engraftment. I?m still sick to my stomach a lot of the time, which I know will increase when I start the new chemo in a few weeks.

The good news is that there is no bad news to report. A year from now I?ll be past Day Plus 300. And who knows? Maybe I?ll be running in the sprinklers.

LAS VEGAS (AP) ? Floyd Mayweather Jr. turned himself in Friday in a Las Vegas courtroom to begin serving a three-month jail sentence for attacking his ex-girlfriend in September 2010 while two of their children watched.

The undefeated five-division champion said nothing as he stood in a gray sweatsuit and sneakers before a judge who credited him with fulfilling community service requirements she imposed in December, then watched as Mayweather was handcuffed and taken away.

Justice of the Peace Melissa Saragosa had allowed Mayweather to remain free long enough to headline a May 5 fight.

Mayweather pleaded guilty in December to reduced domestic battery charges in a hair-pulling, arm-twisting attack on the mother of three of his children. The plea deal avoided trial on felony charges that could have gotten Mayweather up to 34 years in prison if he was convicted.

"He'll be all right," Mayweather friend, rapper 50 Cent, told reporters outside the courthouse.

Las Vegas police say that as a high-profile inmate, Mayweather probably will serve most of his time away from other prisoners in a small solo cell.

Mayweather, who goes by the nickname "Money," will have a cell about one-third the size of a small boxing ring. His stint in the high-rise Clark County Detention Center is expected to limit his ability to train for another fight.

At least for the first week, Mayweather will be segregated for his protection from the other 3,200 inmates in the downtown Las Vegas facility, police Officer Bill Cassell said this week.

Mayweather won't have a TV in his cell, and Cassell said televisions in jail dining areas probably won't carry the June 9 pay-per-view WBO welterweight fight between Mayweather rival Manny Pacquiao and Timothy Bradley at the MGM Grand Garden arena.

The judge sentenced Mayweather on Dec. 22, then later allowed him to remain free long enough to fight Miguel Cotto on May 5 in Las Vegas.

Mayweather was accompanied into the ring by entertainers Justin Bieber and 50 Cent before winning the Cinco de Mayo weekend bout and a guaranteed $32 million. Cotto was paid $8 million.

Saragosa said when she sentenced Mayweather that she was particularly troubled that he threatened and hit ex-girlfriend Josie Harris while their two sons watched. The boys were 10 and 8 at the time. The older boy ran out a back door to get a security guard in the gated community.

However, the judge accepted the deal that had Mayweather plead guilty to misdemeanor domestic battery and no contest to two harassment charges. Prosecutors dropped other charges.

Mayweather's jail stay will be capped at 87 days, because the judge gave him credit for three days previously served. It could be reduced by several weeks for good behavior, Cassell said Thursday. Mayweather also was ordered to complete a yearlong domestic violence counseling program, 100 hours of community service and pay a $2,500 fine.

Harris and the three children now live in Southern California.

Mayweather will be housed in a standard administrative segregation cell no larger than 7-by-12 feet, with a bunk, stainless steel toilet and sink, a steel and wood desk with a permanently bolted stool and two small vertical windows with opaque safety glass.

The cell will be a far cry from Mayweather's nearly 12,800-square-foot, two-story mansion on a cul de sac in an exclusive guarded community several miles south of the Las Vegas Strip. Mayweather's home has two garages, five bedrooms, eight bathrooms, and a swimming pool and hot tub overlooking a golf course.

Mayweather could have about an hour a day out of his cell with access to an exercise yard, Cassell said. Depending on his behavior, the boxer could later get several hours a day for exercise with other inmates also being held in protective custody.

He'll get a standard-issue blue jail jumpsuit with the letters CCDC and orange slippers. Mayweather will be able to deposit money into a jail account to purchase snacks, soap and personal hygiene items from the jail commissary.
